- Please be aware that appointments that are not kept, or we do not receive 24 hours notice to cancel or reschedule can result in a $100 charge.
- If you are new to our Practice, please plan to arrive 15 minutes early to your scheduled appointment time to complete registration documents.
- Please bring any required referrals your insurance requires in addition to all of your medications.
